Please book the appointment using the lactating parent's name exactly as it appears on the insurance card.​

 

After booking, please check your email and complete the forms sent to you. These forms must be completed 1 business day in advance. ​

 

The self-pay rate is a flat-free of $200 for an initial virtual appointment/$275 for initial in-home visit, and $100 for any follow-up appointments.

 

Payment is due at the time of service. Super bill is available if out of network, please note we make no guarantee to any reimbursment by your insurance carrier. ​

 

On a case-by-case scenario, we will travel up to 40 miles from Sykesville, MD.
